Terms & Conditions Born Donor Bank

THE FOLLOWING TERMS AND CONDITIONS SHOULD BE READ CAREFULLY PRIOR TO ORDERING ANY PRODUCTS OR SERVICES FROM THE SPERM BANK, AND PRIOR TO FERTILITY TREATMENT WITH THE DONOR SPERM.
THE TERMS AND CONDITIONS INCLUDE IMPORTANT INFORMATION REGARDING THE DONOR SPERM, SUCH AS RELEVANT MEDICAL INFORMATION, USE OF THE DONOR SPERM AND LEGAL TERMS OF THE AGREEMENT.

The customer may either be:

1) A PRIVATE CUSTOMER
or
2) A PROFESSIONAL CUSTOMER

Re 1)

PRIVATE CUSTOMERS are private citizens who utilize the donor sperm for private use for their own fertility treatment performed by a hospital, a fertility clinic, a doctor, or another healthcare professional. Click here to read the terms and conditions for private customers.

Re 2)

PROFESSIONAL CUSTOMERS are hospitals, fertility clinics, doctors or other healthcare professionals who utilize the donor sperm for professional use, e.g. for fertility treatment of their patients. Click here to read the terms and conditions for professional customers.
